Show Entry Form


This online Show Entry Form was put together because I was tired of filling in entry forms and even on good days my hand writing can be quite illegible. The form is a scanned image of an original, it cost nothing, but time to produce. So why not share it around, it's easy to use and it's FREE.

The form is available in either HTML or PDF format, depending on which browser you are using.

For those wishing to use the HTML version, you must already have Javascript enabled on your browser. Internet Explorer 5.0 or greater click here IE 5.0+ users click here!. This will open a couple of windows, one with the form and the other with the instructions. Unfortunately I haven't quite figured out how to set the form up for viewing with Netscape, but for those wishing to see how it looks (and maybe help me with a solution) click here Netscape users click here, if you must..
